Broker Updates, News & Reminders

1094 / 1095 
1095B forms are sent to employees on an annual basis by Medical Mutual. They are generally sent in mid-February to the employee’s home address. 
1094B forms are filed with the IRS on behalf on the employer. 

Annual Notice of Health Benefit Reminders
As required by ERISA, the Annual Notice of Health Benefit Reminders was sent to all COSE MEWA participating employers in February. It is the employer’s responsibility to share this information with their employees. You can click the following links for the cover letter and accompanying notice:

Sole Proprietor / Group of One Affidavit
The Group of One Affidavit may not be required in all instances. Use these guidelines as a starting point, if there are questions underwriting may follow-up.

Examples of when Group of One Affidavit may not be required:

  • There are W2 wages for an employee other than the owner (ie: part time employees)
  • There is a quarterly wage and tax report available
  • A husband and wife only business where one of them shows up on quarterly report as W2 employee
  • Tax forms provide evidence of full-time business revenue

Admin Reminders

  • Encourage your groups to enroll in paperless billing through EmployerLink
    • Groups that sign-up for EFT withdrawal also avoid the $25 monthly admin fee
    • EFT is withdrawn the first week of the month
    • Groups can view their invoice on EmployerLink and can sign-up for email notification
  • Once enrolled, enrollment changes should be process through EmployerLink or sent to MemApps@medmutual.com (not sent through FormFire or EasyApps)

July 2, 2019

Agreement with New COBRA Vendor Taking Effect Sept. 1, 2019 for Existing Groups

Applies to: Group: 1-50, 51-99, 100+, MEWA

In previous Broker Updates, we announced an agreement with a new COBRA vendor and then a delay in that transition. We have now finalized our agreement and WEX Health will begin administering COBRA benefits for existing groups effective Sept. 1, 2019. The new platform will be called COBRA Options – Brought to you by Medical Mutual and will offer enhanced benefits continuation service and faster setup for new groups.

Transition Plan
An initial letter will be mailed to existing affected groups in July to inform them of this change. The letter will include forms that request group eligibility verification and automated clearing house (ACH) information. These form must be returned to us promptly in order to complete setup on the platform.

We will send a follow-up letter in August to remind existing groups to complete these critical steps.

New Required COSE MEWA Form Available

Applies to: (Small Group (1-50); MEWA (no MA, no MedSupp))

A new form is available to help streamline the process for groups to become part of COSE MEWA: the COSE MEWA Sold Case Pre-submission Review Form. It must be filled out completely and accompany all sold case submission documents to ensure timely processing.

This form can also be found at cosemewa.com, where you will notice a Broker Tools and Resources tab has been added for your convenience. An Order Supplies tool will be available soon, so check the site often for news and important updates. 

COSE MEWA Membership Requirements

As a reminder, COSE and affiliate chamber of commerce memberships are a requirement of the MEWA once a group is approved for enrollment by underwriting. This membership must be renewed annually to avoid termination. Memberships are reviewed monthly, and groups that allow their chamber membership to lapse will receive a 30-day termination notice.

Delay in Transition to New COBRA Vendor for Current Groups
Applies to: Small Group (1-50), 51-99, 100+, MEWA (no Medicare Advantage, no Medicare Supplement)

In a previous Broker Update, we announced that all groups with COBRA benefits would be transitioning to a new vendor platform called COBRA Options - Brought to You by Medical Mutual, effective July 1, 2019. We've recently been informed that only new groups will begin using this platform on that date. There will be a delay in this transition for current groups.

Please note that all group communications will also be delayed. A forthcoming Broker Update will provide a firm transition date for current groups.

Agreement with New COBRA Vendor Taking Effect July 1
(Applies to: Small Group (1-50), 51-99, 100+, MEWA (no MA, no MedSupp))

Beginning July 1, 2019, WageWorks will transition away from administering COBRA benefits for our groups. We have finalized an agreement with WEX Health to become our new COBRA vendor with a platform that will be called COBRA Options - Brought to you by Medical Mutual. Please always refer to the new relationship in this manner going forward. This migration will offer an enhanced platform for benefits continuation services and faster setup for new groups.

These forms need to be returned to us promptly in order to complete setup on the platform. A follow-up letter may be sent if the initial response rate is low.

New Counties Available for Quoting in MedFlex™ Network Primary Service Areas

The following counties have been added as MedFlex™ network primary service areas and are now available for quoting groups beginning with July 1, 2019, effective dates: Delaware, Fairfield, Licking, Pickaway and Madison.

Medical Mutual Announces New Partnership with EasyAppsOnline

Applies to: All group business

Medical Mutual is happy to announce a new partnership with EasyAppsOnline, an application system that is being introduced as an option for online employee medical history questionnaires. These electronic applications are available to all group segments, including Council of Smaller Enterprises (COSE) Health and Wellness Trust. Please note, there will be no changes to Medical Mutual's partnership with FormFire, LLC.

We are excited to announce new tools that will assist in selling the following Medical Mutual Consumer-driven Health (CDH) products:

New CDH Product Fliers for Group Officials

Now available on MyBrokerLink, these print-on-demand fliers highlight the features and benefits unique to the Medical Mutual CDH product line. Share these PDFs via email with groups or print and use them for group presentations

These fliers replace the current group fliers on MyBrokerLink in the Producer's Guide under the HSA FSA HRA Products tab. Please remove the previous PDFs from any saved files and destroy any printed copies.

Also, in response to requests from brokers, we created a new video showing how members can access and manage their HSAs via My Health Plan to My Spending Accounts. The video is available on MyBrokerLink in the Producer's Guide under the HSA FSA HRA Products tab. Please share this video with your groups.

ID Card Release Schedule for New National Network Replacement: New Online Provider Link

This month, Medical Mutual will begin mailing redesigned member ID cards to new and existing members.The cards reflect our new national network, the Aetna® Open Choice® PPO Network. All cards will be mailed prior to Jan. 1, 2019. A member letter will be included with the card informing members of the new card and network, along with information about:

  • Selecting a primary care provider (PCP)
  • Staying in network
  • Using preventive care benefits
  • Knowing what services and medications are covered (for groups with prescription drug coverage through Medical Mutual)
  • How to find our member FAQs document

Updated Provider Search
Our provider search tool has been updated with a link to the national network for 2019. 

As a reminder, members should continue to look up providers in the SuperMed® service area regardless of where they reside.

Consumers Life Renewals
We have exciting news to share regarding Consumers Life renewals. Consumers Life completed a comprehensive review of the Council of Smaller Enterprises (COSE), Toledo Regional Chamber of Commerce (TRCC) and Richland Area Chamber of Commerce (RACC) benefit plans for the upcoming renewal effective July 1, 2018. As a result, current rates will be guaranteed for three (3) additional years. This included an analysis of the incurred loss ratio, demographic information and the overall benefit package.

Participating employers will receive the following communications:

Now – 6/30/2019

  • Participating employers with renewal dates on or after July 1, 2019 will receive a renewal letter within 60 days of their renewal date advising them their rates have been guaranteed for an additional two (2) years.
  • Proposal exhibits prepared for participating employers with effective dates on or after July 1, 2019 will include a two (2) year rate guarantee.

7/1/2019 – Later 

  • Participating employers with renewal dates on or after July 1, 2019 will receive a renewal letter within 60 days of their renewal date advising them their rates have been guaranteed for an additional two (2) years.
  • Proposal exhibits prepared for participating employers with effective dates on or after July 1, 2019 will include a two (2) year rate guarantee.

Medical Mutual Issuing Documents for Autism Spectrum Disorder Treatment 

We are updating our certificate books with updated information for Autism Spectrum Disorder Treatment and notifying affected groups and members.

COSE MEWA Group and Member Notification
We will send a COSE MEWA group official letter which will include the autism spectrum services language contained in the benefit book their employees will receive. Members will receive a letter along with an updated benefit book. The coverage noted became effective January 1, 2018.

As a reminder, this new law is not applicable to all market segments.

COSE MEWA Electronic Funds Transfer (EFT) Form Update 


The COSE MEWA electronic funds transfer (EFT) form has been updated to clarify that a voided check (or deposit slip) is required to verify banking information. The updated EFT form can be accessed HERE.
